Some people mentioned Brother Mhenlol earlier. I'm honored, that's my name. =D Best part is that he's a PvP Rit (Though I'm thinking of changing him to a PvE monk to secure the name in GW2).

Captain No Fun (Necro)
Paragon City (Paragon, City of Heroes reference)
Angorodons Gaze (Mesmer, when Ang's Gaze was at its peak)
Guild Snores (Monk)
Fun With My Scythe (Derv that uses anything BUT a Scythe)
Devona Has Cooties (Hammer War)

One of the best names I saw in the RA was Pwny Slaystation.
